New toolkit aims to boost ACP

The National End of Life Care Programme (NEoLCP) has launched a new toolkit to help care providers facilitate Advance Care Planning with people nearing the end of life.

The kit, entitled 'Advance Care Planning: It all ADSE up’, aims to give care providers the information and guidance they need to feel confident in supporting the Advance Care Planning process.

The toolkit is closely aligned with other NEoLCP materials, including ‘Planning for Your Future Care – A Guide’ (NEoLCP Feb 2012).

Claire Henry, National Programme Director of the NEoLCP, said: “The toolkit provides practical information on resources available to support the Advance Care Planning process. ‘It all ADSE up’ stands for Ask, Document, Share and Evaluate.

“Within the toolkit there are challenges and top tips, as well as links to other useful sources of information and guidance. A series of podcasts are currently being developed to complement the toolkit, and will highlight the benefits of ACP to the individual, their families and carers, as well as the benefits to health and social care staff.”
