What would make hospital end of life care better?

I am interested in others opinon and experiences regarding hospital end of life care. What are the things we can change to improve the patients, carer and staff's experience of end of life care for those patients in hospital? I am also interested in why hospital is often seen as the "place to die"? I feel we can provide an exemplorary end of life care to patients within the acute setting, but why when patients are diagnosed as dying do we deny them of all choice? Why do dying patients in hospital suddenly become static and discharge often cancelled? Can't we discharge to die at home if that is the patients wishes? Do we not advocate home deaths enough??
