Future Matters: End of Life Care Planning Training
The Future Matters four-day course equips participants with the knowledge, skills and tools needed to plan for end of life, help others plan and raises awareness of death, dying and bereavement. Once trained, participants will be able to put their own plans in place, help others with theirs but also run workshops, informal talks, death cafes and deliver presentations to a wide range groups in their community. The course is suitable for community, health and social care professionals, volunteers, those working in complementary therapies as well as individuals wishing to develop their knowledge in end of life care planning.
The training schedule consists of 7 sessions to complete as follows:
Module 1: Introduction to last years of life care planning and Future Matters Peer Education.
Module 2: Putting your house in order Part 1 (Wills & Powers of Attorney)
Module 3: Introduction to Advance Care Planning
Module 4: Putting your house in order Part 2 (end of life support and funeral wishes)
Module 5: Loss and bereavement
Module 6: Planning for Last Years of Life – Peer Education Skills
Module 7: Putting it into practice in the community
