York Blind and Partially Sighted Society

York Blind and Partially Sighted Society is a local charity working to help people with sight problems to be independent. Our mission is to advocate, develop and provide services and facilities which enable people who are blind or partially sighted to achieve independence with dignity, in all aspects of life. As many people lose sight in later life, we work with a large number of older people. People also lose sight due to other conditions such as stroke, MS etc.

York Carers Forum

we are an unpaid carers support group. We have a Monday and Thursay meeting with a drop-in or stay all day with refreshment. Trips for carers and cared for people, former carers (this is important). Some of us attend meetings with the NHS regarding end of life care and unpaid carers issues.

York Hospital Palliative Care Team

North Yorkshire and York PCT have four specialist palliative care teams across a large county, based in York, Northallerton and Scarborough. We provide specialist palliative care advice to patients and their families in hospital and community settings who have a life limiting illness. We are a multi-professional service with family support worker, dietician, occupational therapists as well as doctors and macmillan nurses working together to support end of life care patients.

Yorklea Nursing Home

We are a nursing home in Chorlton Manchester, registered for 31 patients, working very closely with the local PCT. We registered last year to do the gold standard framework.

Young Foundation

The Young Foundation brings together insights, innovation and entrepreneurship to meet social needs. We have a track record of over 50 years' success with ventures such as the Open University, Which?, the School for Social Entrepreneurs and Healthline (the precursor of NHS Direct).

I am the District Nurse for a large team, looking after an Asian population. We do a lot of Palliative care that can at times be very challenging. We aim to support families and patients through the Palliative care process, building a trusting relationship to encourage a positive experience for them.
